Job summary

Bristol Hospice in Cincinnati, OH seeks an experienced Registered Nurse Case Manager to deliver compassionate, patient-centered care along the care journey. As part of a mission-driven team, you will assess needs, develop individualized care plans, monitor vital signs, administer treatments, and collaborate with interdisciplinary colleagues to ensure comfort and dignity for patients and families.

You will educate families, document progress, and advocate for quality end-of-life care.

Qualifications

• Must hold a valid Registered Nurse license in good standing.
• Valid driver's license and reliable transportation.
• Current CPR certification.
• Strong observational and communication skills.
• Flexible, team-oriented attitude.

Responsibilities

• Assess patient needs and develop individualized care plans.
• Deliver one-on-one support to patients and families ensuring comfort and dignity.
• Monitor vital signs and document progress.
• Administer physician-ordered medications and treatments.
• Educate patients and families on care techniques and symptom management.
• Collaborate with interdisciplinary teams to ensure comprehensive care.
• Maintain up-to-date clinical notes reflecting patient progress.
• Contribute to other duties as part of a compassionate team.

Qualifications: On An Average Day You Will:

• Develop and implement individualized care plans based on current and prior medical history
• Deliver attentive, one-on-one support to patients and their families, ensuring comfort, dignity, and understanding
• Conduct thorough assessments of physical, emotional, and spiritual needs throughout the care journey
• Accurately monitor and document vital signs including temperature, pulse, respiration, and blood pressure
• Administer physician-ordered medications and treatments with skill and empathy
• Educate patients and their families on care techniques and symptom management
• Track and communicate changes in patient condition, ensuring timely and effective responses
• Keep detailed and up-to-date clinical notes to reflect patient progress and quality of care provided
• Collaborate with interdisciplinary teams to ensure comprehensive care delivery
• And contribute to other supportive duties as part of a compassionate, mission-driven team
• And additional duties as assigned

What You’ll Bring to The Team:

• A heart for hospice care and a commitment to exceptional service
• A valid Registered Nurse license in good standing in the state you will work in
• A valid driver’s license and reliable transportation
• CPR certification (current)
• Strong observational and communication skills—both written and verbal
