The Registered Nurse (RN) in Home Health provides and oversees nursing care for patients at home, as prescribed by physicians and compliant with laws, regulations, and agency policies.

• Coordinates care plans with healthcare professionals and ensures continuity by communicating with physicians, staff, patients, families, and community resources within a Point of Care setting.
• Conducts initial and/or comprehensive evaluations, develops care plans within state guidelines, and documents accurately and promptly in collaboration with physicians.
• Performs assessments at designated times and develops appropriate care plans.
• Ensures patients meet home health eligibility and medical necessity guidelines as defined by payer sources.

License Requirements:

• Current RN licensure in the state of practice
• Current CPR certification
• Valid driver’s license, vehicle insurance, and access to reliable transportation or public transit
