Job summary

Peninsula Post-Acute in Burlingame, CA is seeking a part-time Registered Nurse (RN) to join our high-acuity post-acute rehabilitation team. AM/PM/NOC shifts are available, and you will provide clinical oversight and direct patient care to residents.

Qualifications

• Current and active California RN license in good standing.

Responsibilities

• Conduct comprehensive nursing assessments and monitor changes.
• Coordinate admissions, discharges, and care planning with the team.
• Support LVNs and CNAs and ensure compliance with policies.
• Maintain accurate EMR documentation per regulations.

Your Impact & Responsibilities

Clinical Oversight

• Provide and oversee nursing care for a designated group of residents.
• Ensure medications and treatments are administered accurately and safely.

Acute Assessment

• Conduct comprehensive nursing assessments.
• Monitor residents for changes in condition and communicate findings promptly to physicians and the interdisciplinary team.

Care Coordination

• Participate in admissions, discharges, and care planning activities.
• Collaborate with residents, families, and healthcare team members to ensure continuity of care.

Team Support

• Provide guidance and support to LVNs and CNAs.
• Promote compliance with facility policies, procedures, and clinical standards.

Documentation

• Maintain accurate and timely electronic medical records in accordance with state and federal regulations.

Qualifications

• Current and active California Registered Nurse (RN) license in good standing.
• Previous experience in a skilled nursing, post-acute, rehabilitation, or sub-acute setting preferred.
• Strong clinical assessment, communication, and organizational skills.
• Ability to thrive in a fast-paced, team-oriented environment.
