Job summary

Elara Caring is seeking a Registered Nurse Home Health to play a pivotal role in delivering compassionate, high-quality in-home nursing care. You will assess and monitor patients, implement plans of care, and educate families to ensure safety and comfort in their homes.

The role emphasizes privacy, clinical excellence, and collaboration with care teams to adapt to patient needs and conditions across assignments.

Qualifications

• Current State license as a Registered Nurse.
• Graduate of an accredited nursing program (certificate/diploma/associates/bachelor).
• 1+ year of clinical nursing experience.
• Home Health experience preferred but not required.
• Ability to lift up to 50 pounds and transfer patients.
• Valid driver’s license or reliable transportation for patient home visits.

Responsibilities

• As a Registered Nurse Home Health, you’ll assess patients to determine care needs.
• Complete initial and ongoing patient assessments.
• Protect patient and staff privacy in accordance with HIPAA.
• Provide nursing care per the plan of care and physician orders.
• Monitor patient conditions and report changes to the care team.

As a Registered Nurse Home Health, you’ll contribute to our success in the following ways:

What is Required?

• Current State license as a Registered Nurse.
• Graduate of an accredited Certificate, Diploma, Associates or Baccalaureate School of Nursing.
• 1+ year of experience as a nurse in a clinical care setting.
• Home Health experience is preferred, but not required.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
