Job summary

Elara Caring in Shawnee, OK, is seeking a Registered Nurse Home Health to provide compassionate in‑home care, complete initial and ongoing patient assessments, and deliver nursing care per the plan of care.

You will educate patients and families on care and safety, monitor conditions, and coordinate with the care team while maintaining HIPAA privacy and safety standards.

Qualifications

• Current State license as a Registered Nurse.
• Graduate of an accredited Nursing program (Certificate, Diploma, Associates or Baccalaureate).
• 1+ year of nursing experience in a clinical care setting.
• Home Health experience preferred but not required.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including transferring patients and moving equipment.

Responsibilities

• Assess patients to determine care needs.
• Complete initial and ongoing patient assessments.
• Protect patient privacy in accordance with HIPAA.
• Provide nursing care according to the plan of care and physician orders.
• Monitor patient conditions and report changes to the care team.
• Educate patients and families on their care and safety.
• Take action when patient conditions change.
• Help ensure quality, safe, and continuous patient care.

What is Required?

• - Home Health experience is preferred, but not required.
• - Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• - Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
