Responsibilities

• Provide skilled nursing care in patient homes, including assessments and treatments.
• Develop and implement individualized home health care plans.
• Monitor patient progress and adjust care plans as needed.
• Educate patients and families on disease management, rehab and senior care.
• Coordinate care with physicians, therapists, and the Director of Nursing.
• Document visits in compliance with home health regulations.
