Job summary

American Home Health is seeking a compassionate Registered Nurse (RN) to join our West LA home health team. You will deliver skilled nursing care in patients' homes and help families manage care plans.

This role requires CA RN license, BLS, and reliable transportation. You will work within an interdisciplinary team to improve patient outcomes and reduce hospitalizations, with flexible part-time scheduling.

Qualifications

As a Registered Nurse (RN), you will provide skilled nursing care to patients in the comfort of their homes, develop individualized care plans, perform comprehensive assessments, administer medications and treatments, educate patients and caregivers, and coordinate care with physicians and the interdisciplinary team. You will play a critical role in helping patients recover safely, manage chronic conditions, and avoid unnecessary hospitalizations.

In this role, you will build meaningful relationships with patients and families while promoting independence, improving outcomes, and ensuring compassionate, patient-centered care.

Must Have Qualifications:

• Current California Registered Nurse (RN) license.
• Current BLS certification.
• Valid driver’s license, reliable transportation, and auto insurance.
• Excellent assessment, communication, and critical thinking skills.
• Ability to work independently while collaborating with an interdisciplinary team.

Nice to Have Qualifications:

• Previous experience working in home health, hospice, or a similar caregiving environment.
• Familiarity with KanTime Electronic Medical Record (EMR) system.
• Bilingual skills are a plus.
