Home Health RN Responsibilities

• Provide skilled home health nursing care, including assessments, wound care, medication management, and treatments
• Develop and implement individualized home health care plans
• Monitor patient progress and adjust care plans accordingly
• Educate patients and families on disease management, rehab, and senior care
• Coordinate care with physicians, therapists, and interdisciplinary teams
• Complete accurate and timely documentation in compliance with home health regulations

Qualifications – RN Home Health Jobs

• Active California Registered Nurse (RN) license (required)
• Minimum 1 year of RN experience (home health experience preferred)
• Experience in home health, rehab, geriatric care, or community care is a plus
• Strong clinical, communication, and organizational skills
• Valid driver’s license and reliable transportation
• Ability to travel within Los Angeles County (Whittier, Lakewood, Santa Fe Springs & surrounding areas)

Preferred Qualifications

• Prior RN Home Health experience
• Public Health Nurse (PHN) certification
• Experience with mobile nursing or home-based care models
