The Medical College of Wisconsin, Inc. is seeking an RN Case Manager to coordinate comprehensive, patient-centered care. You will assess, plan, implement, and evaluate individualized care with interdisciplinary teams, ensuring continuity and efficient resource use.
Responsibilities include telephone triage, care coordination across settings, direct patient care activities, and discharge planning, with emphasis on advocacy and high-quality outcomes.
The Registered Nurse (RN) Case Manager is responsible for coordinating and managing the delivery of comprehensive, patient-centered care. This role ensures optimal patient outcomes by assessing, planning, implementing, and evaluating individualized care plans in collaboration with interdisciplinary teams, patients, and their families. The RN Case Manager serves as a liaison between healthcare providers, community resources, and patients, focusing on effective resource utilization, continuity of care, and patient advocacy.
Appropriate experience may be substituted for education on an equivalent basis.
Work requires occasionally lifting moderate weight materials, standing, or walking continuously.
Little or no exposure to hazards such as dust, fumes, or extreme temperatures, airborne or blood borne pathogens, extreme temperatures, or allergens.
Sensory Acuity Ability to detect and translate speech or other communication required. May occasionally require the ability to distinguish colors and perceive relative distances between objects.
