Job summary

Apple Rehab Farmington Valley is seeking a Registered Dietitian to provide comprehensive nutritional care for residents in our Avon-area facility. You will ensure compliance with food service regulations, and advise Dietary Managers, Nursing, or Administrators as required.

The role includes monitoring resident weights and nutrition intake, participating in weekly At Risk meetings, and making diet and supplement recommendations to support resident health and well-being.

Qualifications

• Registered Dietitian with clinical and production experience in long term care setting.
• Certified by the State of Connecticut as a CD-N.
• Current knowledge of dietary practices, procedures, laws, regulations and guidelines as they apply to long term care.
• Effective communication skills; oral and written.
• Self-motivated and flexible.
• Must demonstrate cooperation; team player approach.
• Must demonstrate patience, tact, enthusiasm, positive attitude.
• Valid driver’s license with vehicle.

Responsibilities

• Provides comprehensive nutritional care for residents in the facility.
• Ensures compliance with Food Service regulations and standards of practice.
• Offers counsel and/or directives to Dietary Managers, Nursing, or Administrators as needed.
• Participates in weekly “At Risk” meetings.
• Monitors resident weights and nutrition intake, and makes diet recommendations.
