Job summary

Apple Rehab in Avon, CT is seeking a Registered Dietitian to deliver comprehensive nutritional care for residents in our facility. You will ensure compliance with regulatory standards and collaborate with Dietary Managers, Nursing, and Administrators.

The role includes monitoring resident weights and nutrition intake, participating in weekly “At Risk” meetings, and making diet recommendations to support residents’ health. A CD-N license and strong communication skills are required.

Qualifications

• Registered Dietitian with clinical and production experience in long term care.
• Certified by the State of Connecticut as a CD-N.
• Knowledge of dietary practices, laws and guidelines as they apply to long term care.
• Excellent oral and written communication.
• Flexible, cooperative and patient-focused.

Responsibilities

• Provide comprehensive nutritional care for residents in the facility.
• Ensure compliance with Food Service regulations and standards of practice.
• Counsel and/or direct Dietary Managers, Nursing, or Administrators as needed.
• Participate in weekly At Risk meetings.
• Monitor resident weights and nutrition intake; make diet recommendations.
