Location: $65,000.00 to $78,000.00 (with benefits; negotiable)

Position Overview

The Grand Rehabilitation & Nursing at Pawling is seeking a Registered Dietitian to provide resident-centered nutritional care in a long-term care and rehabilitation setting.

This role is ideal for an RD who values interdisciplinary collaboration, clinical nutrition, and improving quality of life for residents.

Key Responsibilities

Nutrition Assessment & Planning

• Conduct nutritional assessments and develop individualized care plans

• Monitor intake, weight trends, and clinical outcomes

• Modify dietary plans based on medical conditions and resident needs

Meal Planning & Dietary Oversight

• Develop and oversee therapeutic and balanced meal plans

• Ensure diets align with physician orders and resident preferences

• Maintain consistency across all dietary programs

Education & Collaboration

• Educate residents, families, and staff on nutrition and wellness

• Collaborate with nursing, providers, and interdisciplinary teams

• Provide guidance and training to dietary and kitchen staff

Compliance & Documentation

• Ensure compliance with state and federal regulations

• Maintain accurate nutrition-related documentation

• Support survey readiness and quality improvement initiatives

Qualifications

• Bachelor’s degree in Dietetics, Nutrition, or related field

• Current Registered Dietitian (RD) credential required

• Minimum 2 years of dietitian experience (long-term care or healthcare preferred)

• Strong communication and organizational skills

• Ability to work independently and as part of an interdisciplinary team
