Job summary

Napa Post Acute in Napa, CA is seeking a Registered Dietitian (RD) to join our interdisciplinary healthcare team. You will conduct nutrition assessments, develop care plans, and collaborate with physicians, nursing, and dietary staff to support resident health and recovery.

The role requires an RD/RDN credential, CA licensure eligibility, and strong clinical nutrition knowledge. We offer a full-time, hourly position with competitive pay and opportunities for growth in a modern healthcare

Qualifications

Current Registered Dietitian (RD/RDN) credential required.
Strong knowledge of clinical nutrition and regulatory requirements.
Excellent communication and documentation skills.

Responsibilities

Conduct comprehensive nutrition assessments upon admission and as needed.
Develop, implement, and monitor individualized nutrition care plans.
Evaluate nutritional status and provide recommendations to support resident health and recovery.
Collaborate with physicians, nursing staff, speech therapists, and dietary personnel to ensure optimal resident outcomes.
Monitor weight changes, nutrition risks, and dietary concerns.
Participate in care plan meetings and interdisciplinary team discussions.
Ensure compliance with state, federal, and facility regulations regarding nutrition services.
Provide nutrition education and counseling to residents, families, and staff as needed.
Support the Dietary Services department in maintaining high-quality nutrition programs.
