Job summary

Senior Community Care of Kentucky PACE (Northern) invites a Registered Dietitian to join our Florence, KY team. You will integrate with the IDT to plan and monitor individualized nutrition care, participate in menu planning, and educate participants.

We offer a $5,000 sign-on bonus, competitive pay, and a Monday–Friday schedule with occasional weekends/evenings as needed. Join a mission-driven, care-centered environment focused on seniors in the PACE program.

Qualifications

  • Current registration as a Registered Dietitian (RD/RDN) eligible to practice in Kentucky.
  • Strong communication and customer service skills.
  • Ability to follow care plans and orders approved by the IDT.
  • Must have medical clearance for communicable diseases and up-to-date immunizations before direct participant contact.
  • Must clear background check.
  • Must have a valid driver’s license and transportation.
  • Assess nutritional needs and develop/implement care plans.

Responsibilities

  • Assess nutritional needs, develop and implement nutritional care plans, evaluate and report results.
  • Function as a member of the Interdisciplinary Team; complete assessments with participants and monitor health data.
  • Provide nutritional counseling and education for program participants.
  • Oversee and manage the dining program, including menu planning and serving meals to PACE participants.
  • Assist in monitoring meals, including intake and diet compliance.
  • Attend required in-services and complete on-line modules; represent the department in meetings.

Pay Range: $67,400 to $74,800, Based on experience.

Registered Dietitians

Registered Dietitians play an integral role in maintaining the optimal standard of care for people in our Program of All Inclusive Care for the Elderly (PACE). Not only will you be able to share your specialized knowledge for the nutrition needs of many disease states and allergies, but you will also play a role in food preparation and menu planning, counseling participants, and administering therapeutic diets. You will be involved in growing a start-up PACE -centered on the belief that the well-being of elders with chronic care needs and their families are better served in the community to the maximum degree possible. Bring your expertise and skills as an active member of the IDT to help manage the complex medical, functional, and social needs of older adults in the Northern Kentucky PACE program. This is an exciting, hands-on clinical and program role.

What You’ll Be Responsible For:
  • Assess nutritional needs, develop and implement nutritional care plans, evaluate and report the results appropriately
  • Functions as a member of the Interdisciplinary Team. Completes assessments with participants and monitors weights, medical diagnosis, etc.
  • Provides nutritional counseling and education for program participants.
  • Oversees and manages dining program, from menu planning to helping serve meals to PACE participants in the day center.
  • Assists in monitoring meals, including intake and diet compliance.
  • Attends required in-services and completes assigned on-line modules.
  • Represent the department at various committee meetings as requested by the Center Director and/or the Executive Director. Perform other department duties or special assignments, as assigned.
Required Qualifications:
  • Registered by the Commission on Dietetic Registration of the American Dietetic Association.
  • Verbal and writing abilities necessary to communicate work effectively with various levels of staff, residents, family members and the public, required documentation in medical records.
  • Numerical ability necessary to make various computations in calculating diet patterns, tube feeds, nutritional requirements and assisting the Director of Dietary in his/her responsibilities.
  • Ability to utilize computers and other electronic devices for tasks such as timekeeping, in-servicing and documentation.
  • Minimum of one year clinical nutrition experience in a geriatric healthcare setting.
