Resolve Diamondback is seeking a Full-Time Registered Dietitian to join our interdisciplinary healthcare team. You will perform nutrition assessments, develop care plans, and deliver counseling to residents and families. Qualifikationen

• Current licensure as required by state regulations.
• Experience in skilled nursing, long-term care, rehabilitation or healthcare settings preferred.
• Knowledge of clinical nutrition, therapeutic diets, and regulatory requirements.
• Strong communication and organizational skills.
• Ability to work independently while collaborating with interdisciplinary teams.

Aufgaben

• Conduct nutrition assessments and screenings for residents.
• Develop, implement, and monitor individualized nutrition care plans.
• Provide nutrition counseling and education to residents, families and staff.
• Monitor resident weights, nutritional status, and dietary interventions.
• Collaborate with physicians, nursing staff, therapy teams, and dietary services.
• Ensure compliance with state, federal, and facility regulations.
• Participate in care plan meetings and interdisciplinary team discussions.
• Review menus and dietary programs to support resident needs.
• Maintain accurate and timely documentation.
