Healthcare Services Group, Inc in Connecticut is seeking a Registered Dietitian to manage clinical nutrition services at a long-term care facility. You will assess residents, develop care plans, and collaborate with the interdisciplinary team to improve health outcomes.

This role offers Monday - Friday schedules with flexibility and covers multiple buildings as needed. Eligible candidates must be registered with the Commission on Dietetic Registration and hold an active state license.

Qualifications

• Registered Dietitian with CRD registration is required.
• State licensure in good standing is required; multi-site eligibility is a plus.
• Knowledge of CMS/state regulations related to nutrition care is preferred but not required.

Responsibilities

• Assess residents' clinical nutrition needs and document plans in the electronic medical record.
• Develop, implement, and evaluate individualized care plans based on findings and weight trends.
• Provide nutrition education and counseling to residents, families, and staff.
• Collaborate with interdisciplinary teams to improve health outcomes.
• Ensure adherence to CMS, state, and federal regulations and company standards.
• Provide coverage across multiple buildings as needed.
