Autumn Woods Healthcare Facility is seeking a Registered Dietitian to join our team. The RD will assess and plan the nutritional care of residents in accordance with applicable federal, state, and local standards, coordinating with the Food Service Director to ensure quality nutrition and meals.

The role emphasizes documentation, collaboration with the care team, and adherence to regulatory guidelines. We offer comprehensive benefits and career growth within our facility.

Qualifications

• Bachelor's degree in Dietetics, Nutrition, or related field.
• Registered Dietitian with the Commission on Dietetic Registration.
• License or certification as mandated by the state.
• ServSafe Food Safety Certification preferred.
• Experience in long‑term care and geriatric nutrition preferred.

Responsabilités

• Maintains clinical nutrition documentation for residents per policy and regulations.
• Ensures accurate and descriptive progress notes in medical records.
• Participates with interdisciplinary team in planning and evaluating care plans.
• Coordinates with Food Service Director/Manager to customize menus to resident needs.
• Assists in providing and reviewing diet orders and ensuring tray cards are accurate.
