Responsibilities

Assess residents’ nutritional status and develop individualized care plans.
Monitor and evaluate dietary interventions and adjust as needed.
Collaborate with interdisciplinary teams including nursing, therapy, and medical staff.
Ensure compliance with state and federal regulations, including survey readiness.
Provide nutrition education to residents, families, and staff.
Oversee menu planning to ensure balanced, appealing, and therapeutic diets.
Participate in care plan meetings and quality improvement initiatives.
