Job summary

Healthcare Services Group Inc. is seeking a Registered Dietitian to manage clinical nutritional services at a long-term care facility in Indiana. Monday – Friday with flexible schedules and a competitive pay range.

You will assess residents’ nutrition needs, develop, implement and document care plans, provide nutrition education to residents, families and staff, and collaborate with interdisciplinary teams to improve health outcomes while ensuring CMS/state compliance.

Qualifications

Registered by the Commission on Dietetic Registration.
State Certification/License in good standing.
Knowledge of state survey process (preferred).
Strong organizational and communication skills.
Compliance with vaccination policies as required.
Driver's license if multi-site RD.

Responsibilities

Assess the clinical nutrition needs of residents/patients and document the plan in the EMR.
Develop, implement, and evaluate individualized care plans based on clinical findings, lab results, weight trends, intake, and comorbidities.
Provide nutrition education and counseling to residents/patients, families, and staff.
Collaborate with interdisciplinary teams to determine a plan of care that improves residents'/patients’ overall health and well-being.
Ensure adherence to CMS, State, and Federal regulations and company standards.
Provide coverage in multiple buildings based on company needs, and as desired.
