Job summary

Healthcare Services Group (HCSG) is seeking a Registered Dietitian to manage clinical nutritional services at a long-term care facility. You will work Monday–Friday with flexible schedules, delivering nutrition assessments, care plans, and education to residents, families, and staff.

The role requires RD credential, state licensure, and strong organization/communication skills, with compliance to vaccination policies.

Qualifications

• Registered by the Commission on Dietetic Registration.
• State license in good standing.
• Knowledge of state survey process (preferred).
• Strong organizational and communication skills.
• COVID-19 vaccination policy compliance.

Responsibilities

• Assess the clinical nutrition needs of residents/patients and document the plan within the electronic medical record.
• Develop, implement, and evaluate individualized care plans based on clinical findings, labs, weight trends, intake, and comorbidities.
• Provide nutrition education and counseling to residents, families, and staff.
• Collaborate with interdisciplinary teams to determine a plan of care that improves residents'/patients’ health.
• Ensure adherence to CMS, State, and Federal regulations and company standards.
• Provide coverage in multiple buildings based on company needs, and as desired.
• All other duties as assigned.
