Job summary

Qualifications
- Registered dietitian credentials required.
- Licensure in good standing for the state of practice.
Responsibilities
- Assess the clinical nutrition needs of residents/patients and document the plan in the EMR.
- Develop, implement, and evaluate individualized care plans based on clinical findings, labs, weight trends, intake, and comorbidities.
- Provide nutrition education and counseling to residents/patients, families, and staff.
- Collaborate with interdisciplinary teams to determine a care plan that improves residents' health and well-being.
- Ensure adherence to CMS, State, and Federal regulations and company standards.
- Provide coverage in multiple buildings based on company needs, and as desired.
- All other duties as assigned.
