Job summary
Healthcare Services Group, Inc. is seeking a Registered Dietitian to manage clinical nutritional services at a long-term care facility.
You will assess residents, develop care plans, provide nutrition education, and collaborate with interdisciplinary teams to improve health outcomes. Qualifications
- Registered by the Commission on Dietetic Registration.
- Licensure in good standing in the state of practice.
- RD-Eligible and provisional licensure accepted in certain locations.
- Knowledge of state survey process and compliance with nutrition care regulations (preferred).
Responsibilities
- Assess the clinical nutrition needs of residents/patients and document the plan in the EMR.
- Develop, implement, and evaluate individualized care plans based on clinical findings, labs, weight trends and comorbidities.
- Provide nutrition education and counseling to residents/patients, families, and staff.
- Collaborate with interdisciplinary teams to determine a plan of care that improves health and well-being.
- Ensure adherence to CMS, State, and Federal regulations and company standards.
- Provide coverage in multiple buildings based on company needs.
- All other duties as assigned.
