Job summary

Healthcare Services Group (HCSG) is seeking a Registered Dietitian to manage clinical nutrition services in a long-term care facility. You will assess needs, develop care plans, educate residents and families, and coordinate with interdisciplinary teams to improve health outcomes.

Qualifications

• Registered by the Commission on Dietetic Registration.
• State license in good standing in the state of practice.
• Knowledge of state survey process and compliance with nutrition care regulations.
• Strong organizational and communication skills.
• Communication with residents, families, staff, and administration to execute daily tasks.
• Compliance with COVID-19 vaccination policies as required.
• RD-eligible or provisional licensure accepted in certain locations.
• If multi-site RD, valid Driver's License.

Responsibilities

• Assess the clinical nutrition needs of residents and document the plan in the EMR.
• Develop, implement, and evaluate individualized care plans based on clinical data.
• Provide nutrition education and counseling to residents, families, and staff.
• Collaborate with interdisciplinary teams to improve residents' health and wellbeing.
• Ensure adherence to CMS, state, and federal regulations and company standards.
• Provide coverage in multiple buildings as needed by the company.
• All other duties as assigned.
