Outfield-Healthcare-Partners is seeking a Registered Dietitian to provide nutrition services to skilled nursing centers in the Indianapolis area. You will complete assessments, implement nutrition care plans, monitor outcomes, and collaborate with nursing to optimize patient care.

Site visits and menu integration with facility services are included. Ideal candidates have strong communication, ability to work independently and in teams, knowledge of long-term care regulations, and HIPAA

Competenze

• Registered Dietitian with a bachelor's degree and licensure; experience in long-term care or similar setting.
• Ability to travel locally and maintain HIPAA confidentiality.
• Knowledge of nutrition care process, TPN/enteral nutrition calculations.

Mansioni

• Conduct nutritional assessments for new admits and residents with weight changes.
• Calculate TPN and enteral nutrition for residents in long-term care.
• Provide dietary services following state/federal laws and corporate guidelines.
• Educate staff; conduct in-service training and regulatory compliance.
• Document rounds and manage survey readiness.
