Community Health Network was created by our neighbors, for our neighbors. Over 60 years later, “community” is still the heart of our organization. It means providing our neighbors with the best care possible, backed by state-of-the-art technology. It involves getting involved in the communities we serve through volunteer opportunities and benefits initiatives. It also means ensuring our dedicated caregivers can learn and grow to stay at the top of their fields and to better serve our patients. Above all, it means delivering exceptional care — simply and effectively.
The Registered Dietitian performs professional clinical nutrition patient care functions for patients on assigned units. They maintain effective communication within the department and with other healthcare team members to provide quality nutritional care. They also provide nutrition education for the department, facility, and community. As a member of the multidisciplinary healthcare team, they implement nutrition care plans and plan therapeutic diets in accordance with physicians' diagnoses and regulatory requirements.
At Community Health Network, we build teams that deliver exceptional care through empathy, communication, and collaboration. We consider all our employees as Caregivers, integral to the patient experience. We pride ourselves on fostering a supportive environment where caregivers can make a difference in their community.
