Purpose and Scope
Provides nutritional services for the facility’s in-center and home patients to maximize nutritional status and improve clinical outcomes. Supports the FMCNA commitment to QEP and QAI activities, actively participating in process improvements to help patients achieve their individualized goals as determined by their physicians.
Principal Duties and Responsibilities
- Assess patients' knowledge of diet and kidney disease; provide education tailored to their learning needs.
- Provide detailed, patient-specific education on nutritional status, lab parameters, and disease impact.
- Complete nutritional assessments and participate in interdisciplinary care planning.
- Develop individualized care plans in accordance with federal and state guidelines.
- Calculate diet prescriptions based on standard practices.
- Document progress notes and maintain accurate medical records.
- Offer ongoing nutrition assessments and counseling to support effective nutritional status.
- Identify malnutrition risks and collaborate on interventions, reviewing lab results monthly.
- Monitor adherence to nutrition therapy, adjusting as needed, and document responses.
- Collaborate on recommendations for urea kinetic modeling, mineral metabolism, and other clinical areas.
- Adhere to FMCNA policies and participate in quality improvement programs.
- Coordinate patient education and referrals for nutritional supplements as appropriate.
- Review lab results, counsel patients, and recommend treatment modifications.
- Communicate effectively with physicians and facility staff regarding patient labs and treatment plans.
- Participate in staff meetings and collaborate with the corporate dietitian.
- Maintain professional knowledge and credentials, including dietetic registration and continuing education.
- Ensure safety and cleanliness in work areas, and perform other duties as assigned.
Physical Demands and Working Conditions
- Work environment typical of healthcare settings, with possible exposure to infectious materials.
- Includes desk work, patient interaction, and travel between facilities.

Education and Credentials
- Registered Dietitian credential from the Commission on Dietetic Registration.
- Board Certified Specialist in Renal Nutrition preferred.
- State licensure if applicable.
Experience and Skills
- At least 1 year of professional clinical nutrition experience as a Registered Dietitian.
- Renal experience preferred.
- Strong leadership, organizational, communication, and team skills.
- Ability to work independently with minimal supervision.
- Basic computer skills required.
