Job summary

Nutrition that Works, LLC is seeking a consultant dietitian to join our team in West Springfield, MA for 16 hours per week. The role offers a flexible schedule and competitive pay while serving our long-term care community.

Responsibilities include completing clinical nutrition assessments, documenting with the Nutrition Care Process in the EMR, and collaborating with the care plan team. Licensure and CDR credentialing are required, with a client-centered, communicative approach preferred.

Qualifications

• Must hold CDR credential and dietetic licensure.
• Demonstrates client-centered approach and strong communication.
• Positive attitude and collaborative mindset for care planning.

Responsibilities

• Complete clinical nutrition assessments.
• Document using the Nutrition Care Process in the EMR.
• Collaborate with the interdisciplinary care plan team.
• Complete the MDS - Section K.
• Calculate TF.
• Provide wound and weight-change recommendations.
