Job summary

PACS is a national post-acute healthcare network delivering mission-driven nutrition and care across multiple facilities.

We seek a Registered Dietitian to lead nutritional assessments, manage MDS entries, and collaborate with nursing, therapy, and dietary teams to optimize resident meals while ensuring CMS readiness in a fast-paced skilled nursing setting. You will participate in care planning, monitor high-risk residents, and contribute to quality improvement initiatives.

Qualifications

  • Must be a Registered Dietitian (RD) or RDN certified by the CDR.
  • Hold a current state license or certification as required by the state of practice.
  • 1–2 years clinical experience in long-term care, sub-acute, or skilled nursing preferred; new grads considered with strong geriatrics.
  • Proficiency in calculating enteral nutrition regimens and recognizing signs of malnutrition.
  • Experience using LTC software (PointClickCare, MatrixCare) and performing MDS entries.

Responsibilities

  • Nutritional Assessments: comprehensive evaluations on admission, quarterly, annually, and with status changes using standardized tools (MDS 3.0 K).
  • High-Risk Resident Management: develop and monitor interventions for weight loss, tube feeding, skin integrity, and dialysis.
  • Interdisciplinary Care Planning: participate in weekly care team meetings with nursing, SLP, physicians, and PTs.
  • MDS Documentation: timely documentation of nutritional status and interventions in EHR and MDS system.
  • Food Service Collaboration: review menus with Dietary Manager to ensure nutritional adequacy and sanitation standards.
  • Regulatory Compliance: align nutrition care with CMS F-Tags for surveys.

Job description

Core Responsibilities & Duties
  • Nutritional Assessments: Complete comprehensive nutrition evaluations upon admission, quarterly, annually, and during significant changes in status, utilizing standardized tools (e.g., MDS 3.0 Section K).
  • High-Risk Resident Management: Develop, implement, and monitor specialized clinical interventions for high-risk residents, specifically targeting involuntary weight loss, tube feedings (enteral nutrition), altered skin integrity (pressure injuries), and dialysis.
  • Interdisciplinary Care Planning: Actively participate in weekly interdisciplinary care team meetings, collaborating with nursing, speech therapy (SLP) for dysphagia management, physicians, and physical therapists.
  • MDS Documentation: Ensure timely and accurate documentation of resident nutritional status and interventions within the Electronic Health Record (EHR) and MDS system.
  • Food Service Collaboration: Work closely with the Dietary Manager to review menus, ensure liberalized diet plans meet nutritional adequacy, check food palatability, and maintain high standards of sanitation and food safety.
  • Regulatory Compliance: Maintain facility readiness for state and federal surveys by ensuring all clinical nutrition care practices align with CMS F-Tags (e.g., F692 for Nutrition/Hydration, F693 for Tube Feeding).
Required Skills & Qualifications
  • Active Credential: Must be a Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN) certified by the Commission on Dietetic Registration (CDR).
  • State Licensure: Hold a current state license or certification as required by the state of practice.
  • Experience: Minimum of 1–2 years of clinical experience in a long-term care, sub-acute, or skilled nursing setting preferred. New graduates with strong clinical rotations in geriatrics will be considered.
  • Specialized Knowledge: Proficiency in calculating enteral nutrition regimens, identifying signs of malnutrition, and managing complex geriatric disease states.
  • Technical Skills: Experience using long-term care software (e.g., PointClickCare, MatrixCare) and executing MDS entries.
Physical Demands & Work Environment Setting

Fast-paced skilled nursing and rehabilitation facility.

Physical Effort: Requires frequent walking, standing, and sitting to conduct resident assessments across multiple units.

Salary: $40/hour
