Job summary

Lakeport Post Acute, LLC is seeking a dedicated Dietitian/Nutrition Specialist to oversee comprehensive nutritional care for residents in a post-acute setting. You will conduct initial, quarterly and annual reviews, adjust plans, and monitor dietary safety and quality in collaboration with the Dietary Supervisor.

The role emphasizes developing individualized care plans, counseling residents, and ensuring adherence to state and federal guidelines while supporting a multidisciplinary team.

  • Consult with physicians and other healthcare personnel to determine diet restrictions.

Job description

Complete nutritional initial, quarterly, annual and significant change reviews on residents according to federal and state guidelines. Assist in coordination of nutrition care services with Dietary Supervisor. Complete nutritional reviews monthly on high risk residents (significant weight loss/gain, pressure ulcer, hemodialysis, and tube fed).

  • Ability to meet all health, compliance, and competency requirements.
  • Attends and participates in morning meetings/stand up to facilitate communications with the team.
  • Assess nutritional needs, diet restrictions and current health plans in order to develop and implement dietary care plans and provides nutritional counseling as needed.
  • Monitor food services operations to ensure conformance to nutritional, safety, sanitation and quality standards, as well as state and federal regulations.
  • Monitor food control systems such as food temperatures, portion control, preparation methods, garnishment and presentation of food in order to ensure that food is prepared and presented in an acceptable manner.
  • Inspect diet trays for conformance to physician's diet orders prior to delivery.
  • Consult with physicians and other health care personnel (Dialysis Dietitians, Nurse Practitioners, etc.) to determine diet restrictions and nutritional needs of residents.
  • Ability to relate positively, effectively, and appropriately with patients/residents, families, staff and professional colleagues.
