Job summary

Care Guide Partners PACE in Elizabethtown, KY is hiring a Registered Dietitian to screen and assess participants' nutritional status, plan interventions, and communicate findings to the interdisciplinary team to support at-home aging.

You will coordinate meals, supervise kitchen operations, collaborate with contracted meal services, and provide nutrition education to participants and healthcare professionals; travel within the service area and CPR certification are required.

Qualifications

• Currently licensed Registered Dietitian in Kentucky.
• Minimum 1 year of experience with frail or elderly populations or training upon hire.
• Must maintain CPR certification.
• Must have flexibility to travel within the service area.
• Proficiency with technology-based tools, including EMR.

Care Guide Partners PACE is a part of Hosparus Health. As a Program of All-Inclusive Care for the Elderly (PACE) provider, we work to empower people 55 and older with the support they need to stay in the homes they know and love as they age. PACE helps seniors who need nursing home-level care maintain their independence by providing all the medical care and support services they need to continue living comfortably and safely at home.

Schedule: Monday-Friday 8:30-5:00pm

Position Summary:

The Registered Dietitian screens and assesses participant nutritional status. Uses pertinent data to plan and implement appropriate nutrition interventions and communicates the information to the Interdisciplinary Team to ensure the nutritional needs of the participants are met. Performs nutritional assessment and provides nutritional education for the participants requiring interventions. Coordinates serving of meals; monitors/coordinates kitchen facilities to ensure standards are met. Coordinates with contracted meal services to ensure meals meet the needs of the PACE participants. Provides education to other health care professionals as appropriate.

Position Qualifications:

• Minimum 1 year of experience working with a frail or elderly population or must receive appropriate training upon hire.
• Must maintain CPR certification.
• Must have flexibility to travel within the service area.
