Habitat Health in California is seeking a licensed Dietitian to join our multidisciplinary team serving older adults in PACE programs. You will assess nutritional status, develop care plans, educate participants and caregivers, and coordinate with the PCP and care team to optimize meals and supplements.
The role requires a Master’s degree in nutrition/dietetics, RD credential, and at least one year of experience with older adults.

Role Scope:
We are looking for a dynamic, service-oriented dietitian who is passionate about supporting elderly persons in their journey. The Dietitian will work within a multidisciplinary care team led by a primary care provider to ensure whole-person, integrated care. The Dietitian completes participant nutritional assessments and reassessments, screens and assesses participant’s nutritional status and uses pertinent data to plan and implement appropriate nutrition interventions, obtains participant food preferences and makes dietary modifications as needed, in addition to providing nutrition education to participants, caregivers and Habitat PACE staff.

Compensation:
The anticipated base salary range for this position is $89,000 - $105,000 . This position is also eligible for a target annual bonus of 5% and may be eligible for additional incentive compensation, as applicable.
The final compensation offered will be determined based on factors including, but not limited to, the candidate's qualifications, skills, experience, education, certifications, geographical location, and business needs.
In addition, this position is eligible for a comprehensive benefits package, including medical, dental, and vision insurance; paid parental leave, short- and long-term disability coverage; life insurance; flexible spending accounts; a 401(k) savings plan that includes a match; paid time off; and company paid holidays.
