• Evaluates nutrition status and screens for nutritional risk per protocols.
• Plans and initiates medical nutrition therapy as indicated.
• Educates and counsels patients and families on nutrition.
• Communicates care plans to the health care team.

Responsibilities

• Evaluates and assesses nutrition status of patients, and screens patients for nutritional risk in accordance with established department protocols and standards.
• Plans and initiates medical nutrition therapy to patients if indicated by patient requirements and clinical protocol.
• Educates and counsels patients and families on nutritional issues, either individually or in groups
• Communicates individual nutritional care plans to other health care providers as a member of a health care team.

Skills

• Must be a Registered Dietician
• A minimum of two years experience in related field
• Certified in safety and sanitation in regards to food service
• Experience with performing clinical assessments on residents, meeting with Care Plan Team, performing input on MDS
