Job Description

In this role, you will evaluate patients’ nutritional status, plan, coordinate, and manage their nutritional care. You will build relationships with physicians and nursing staff to ensure a standard of care that meets patients' nutritional needs and goals.

Responsibilities

• Conduct comprehensive nutritional assessments by collecting, recording, and interpreting data from patients, family members, caregivers, and others
• Formulate nutrition intervention plans based on identified concerns
• Collaborate with healthcare providers, patients, and caregivers to implement and communicate the nutrition care plan and expected outcomes
• Provide nutritional counseling and education to patients, families, and caregivers
• Maintain accurate records of daily clinical activities

Qualifications

• Bachelor’s Degree in Dietetics, Nutrition, or a related field
• Registered Dietitian credentials
• Two years of clinical/practical experience; geriatrics experience preferred
• Excellent verbal, written, and oral communication skills
