What is Remote Patient Monitoring?

Remote patient monitoring (RPM) uses digital tools to track patient health data from outside the healthcare setting. Patient data is collected from wherever the patient is located & electronically transmitted (securely) to their healthcare providers. RPM helps reduce healthcare costs by increasing in-home support, patient education, and accountability, and by preventing relapse or readmission to hospitals.

About Us

RPM Healthcare is a fast-growing company developed by a passionate, talented entrepreneurial team with nearly 40 years of patient education experience. Our team has created an application called RPM365 used to continuously track patients with chronic conditions like asthma, COPD, diabetes, hypertension, etc. The application identifies out-of-range values that can be dangerous and notices trends or triggers impacting a patient's medical condition. To follow up on patients over time and provide personalized care, an RPM365 Care Coach is assigned to support, engage, and track the patient's data, which is then received by the patient's healthcare provider.

About You

If you are a Registered Dietitian interested in serving patients remotely, this may be the opportunity you are looking for. RPM Healthcare seeks individuals passionate about improving healthcare delivery, leveraging technology for better patient outcomes, and capable of working in a rapidly evolving environment. This position requires being tech-savvy and comfortable with phone outreach. As an RPM365 Coach, you will work as an independent contractor (no benefits offered), engaging in telephonic outreach and building trusting relationships with patients. You should be caring, patient, persistent, understanding, respectful, flexible, motivated, and a self-starter.

Duties and Responsibilities

You will be part of a growing team working to enhance the home-based healthcare experience, representing RPM Healthcare and serving as a trusted point of contact for patients and their medical providers. Responsibilities include:

  • Serving as the primary contact for patients enrolled in the RPM365 platform
  • Assessing patients' nutritional status, goals, and barriers
  • Helping patients achieve their goals by identifying hurdles and providing solutions, teaching behavioral regulation techniques, and facilitating personal growth
  • Engaging regularly with patients to encourage adherence to treatment plans and data monitoring
  • Educating patients through shared information such as articles and news
  • Attending virtual team meetings for updates
Minimum Qualifications
  • RD license
  • Knowledge of chronic health conditions and operation of related devices (glucose monitor, blood pressure cuff, pulse oximeter, spirometer, scale, etc.)
  • Knowledge of weight management
  • Proficiency with technology and comfort navigating computer platforms and smartphone functionalities
  • Experience with client interaction and excellent written and verbal communication skills
  • Highly organized, efficient, a team player, and self-motivated with a strong sense of ambition and discipline
Preferred Additional Qualifications
  • Telehealth experience
  • Strong technology skills
  • Patient monitoring and education experience
  • Additional language skills are a plus
