Job summary

The Paramount at Somers Rehabilitation and Nursing Center is actively seeking a Dietetic Technician, Registered (DTR) to join our dedicated team. You will perform nutrition assessments and provide diet education, while collaborating with the interdisciplinary team to support residents’ care plans.

Responsibilities include attending care plan meetings, ensuring each plan is carried out, and assisting with discharge planning.

Qualifications

• Must be a Dietetic Technician, Registered and eligible to work in the United States.
• Must have at least 1 year of healthcare experience; nursing home experience is a plus.
• Must be able to complete nutrition assessments and provide diet education.
• Must participate in care plan meetings and rounds for residents.
• Proficient with EMR; knowledge of MealTracker is a plus.
• Able to type and use computer programs (Word, Excel, Google Docs).

Responsibilities

• Conduct nutrition assessments and provide diet education to residents.
• Attend care plans and rounds with the interdisciplinary team.
• Ensure the resident’s plan of care is followed and updated as needed.
• Assist with discharge planning and documentation.

The Paramount at Somers Rehabilitation and Nursing Center is looking for an ambitious and dedicated Dietetic Technician, (DTR) to join our ever-growing team.

Dietetic Technician, Registered (DTR)Job Summary:

• Must have 1 year of healthcare experience, nursing home experience a plus.
• Must be able to work well with the interdisciplinary team, exhibit strong communication skills, as well as work independently on daily tasks
• Must be able to complete nutrition assessments and provide diet education
• Must be able to attend care plans of assigned residents with clinically complex diagnosis, as well as other meetings and rounds for the facility.
• Ensure resident's plan of care is in place and carried out in its entirety.
• Assist with discharge planning.
• Must be proficient in EMR, knowledge of point click care a plus.
• Knowledge of MealTracker is a plus.
• Must be able to type & use computer programs proficiently (i.e., Microsoft Word, Excel, Google Docs, etc).
