Job summary

Dartmouth-Hitchcock Health is seeking a Care Manager - Regional Float to support care coordination across six DH system hospitals, including Lebanon and surrounding facilities. The role involves interprofessional collaboration, assessment, and placement planning to ensure seamless transitions and optimal patient outcomes.

Requires RN licensure in NH with eligibility, MSN plus 3 years of clinical experience, and current BLS. Regular schedule with 12-hour shifts and weekend rotation apply.

Qualifications

• RN licensure required or NH eligibility
• MSN with at least 3 years of clinical nursing experience
• Current BLS certification required

Responsibilities

• Coordinate interdisciplinary care across the care continuum for defined populations.
• Develop comprehensive care plans addressing clinical, psychosocial, and environmental needs.
• Advocate for patient access and negotiate services within various health plans.
• Monitor patient transitions across care settings and ensure continuity of care.
• Share care plan data with patient/family with consent as care moves.
• Identify gaps in care and collaborate with networks to expand access to services.
• Contribute to disease management strategies and measure outcomes.
• Develop and maintain an interdisciplinary care delivery system for individual patient needs.
• Collect and evaluate clinical and financial outcomes including patient satisfaction and resource use.
• Perform other duties as required.
