Job summary

Qualifications

• Must be a Registered Dietitian with CDR.
• Must hold a state license in the state of applying, if indicated.
• 1 year experience preferred.

Responsibilities

• Complete timely nutrition assessments, follow-ups, and documentation per CMS and facility policies
• Develop and implement individualized nutrition care plans aligned with physician orders and resident goals
• Monitor residents at nutritional risk, including weight changes, wounds, tube feeding, and altered intake
• Collaborate with nursing, therapy, food service team, and medical providers to ensure coordinated care
• Ensure diet orders are accurate, implemented correctly, and communicated effectively to the food service team
• Support survey readiness through compliant documentation and participation in audits as needed
• Provide education to residents, families, and staff as appropriate
• Participate in Quality Assurance and Performance Improvement (QAPI) activities
