Sweetwater Care is seeking a Registered Dietitian to lead the nutrition services for our senior participants. You will oversee assessments, care plans, and menu planning while mentoring nursing and dietary staff in a collaborative team environment.

The role emphasizes data-driven decision making, adherence to regulatory standards, and the ability to work independently within a skilled nursing setting.

Qualifications

• RD credential required or to be obtained by start date.
• Bachelor's degree preferred, in nutrition, dietetics or a related field.

Responsibilities

• Conduct comprehensive nutrition assessments and participate in IDT meetings.
• Deliver and document nutrition interventions per physician orders and care plans.
• Oversee menu planning and meal delivery to ensure safety and diet accuracy.
• Coordinate supplements as ordered.
• Provide staff training on meal delivery program.
• Manage nutrition staff including hiring, training, supervising and mentoring.
• Support intake and enrollment processes to enable timely enrollment.
• Update IDT promptly on changes in participants' condition or psychosocial status.
• Maintain confidentiality and adhere to policies and procedures.
