Job summary

The University of Vermont Health Network's Regional Transfer Center seeks a Nurse Coordinator to ensure safe, timely coordination of patient care across the network and region.

As a Level 2 Nurse Coordinator, you will field physician transfer calls, coordinate emergent and specialty treatment, and act as a traffic controller to support patient flow in network facilities. You will also oversee transfer backs and report at the Network Daily Pulse Check.

Qualifications

• RN licensure required (state of Vermont or compact)
• Minimum 2 years of RN experience required

Responsibilities

• Coordinate care transfers by fielding calls from physicians and coordinating emergent or specialty treatment.
• Manage patient flow across network facilities using critical thinking and collaboration.
• Oversee transfer backs and report during Network Daily Pulse Check.
