Qualifications

• Current RN licensure or compact licensure recognized by the state of Vermont is required.
• A minimum of 2 years of RN experience is required.

Responsibilities

• Fields calls from physicians for care coordination across the network.
• Coordinates emergent, urgent, or specialty treatment with care providers and regional hospitals.
• Acts as a traffic controller to support patient flow in network facilities in collaboration with local resources.
• Oversees and coordinates all transfer backs and reports at the Network Daily Pulse Check.
