Job summary

Mosaic Management is seeking a Regional Nurse to provide clinical leadership across multiple Oregon communities, with travel required. You’ll mentor teams, focus on resident-centered care, and lead quality and regulatory initiatives.

The role emphasizes improving outcomes, educating staff, and partnering with regional leaders to elevate care quality across assisted living and memory care settings.

Qualifications

• Current Oregon RN or LPN license with senior housing experience.
• Experience in Assisted Living, Memory Care, Long-Term Care, or geriatrics.
• Strong knowledge of Oregon Assisted Living regulations and survey processes.
• Experience preparing for regulatory surveys.
• Exceptional leadership and coaching abilities.
• Strong organizational and problem-solving skills.
• Excellent communication and interpersonal skills.
• Ability to travel regularly throughout Oregon.

Responsibilities

• Provide clinical oversight and support to community leadership and nursing teams.
• Assist communities in delivering resident-centered care.
• Review clinical documentation and resident outcomes.
• Develop individualized care plans and clinical best practices.
• Utilize QA tools to monitor clinical performance.
• Prepare communities for state surveys and inspections.
• Conduct mock surveys and identify opportunities for improvement.
• Ensure compliance with state, federal, and local regulations.
• Assist in developing policies, procedures, and corrective action plans.
• Monitor quality indicators and clinical outcomes.
• Train and mentor nursing and caregiving teams.
• Educate on infection prevention, medication management, resident safety, wound care, regulatory requirements.
• Support onboarding and ongoing clinical development.
• Partner with Executive Directors, Health Services Directors, and regional leadership.
