Job summary

Millennium Physician Group in Florida is seeking a Regional Medical Director to lead physician and APP performance within value-based care models and coordinate care delivery across the designated region. You will liaise with SNFs, hospitals, and specialists to ensure high-quality, cost-effective care.

Reporting to the Chief Medical Officer in the Florida region, you will travel regionally up to 40% and guide clinical strategies, education, and program development to improve population health

Qualifications

• MD or DO with active Florida medical license.
• Board certification in a primary care specialty preferred.
• Minimum of 5 years clinical experience; prior leadership in value-based care setting strongly preferred.
• Demonstrated success in managing provider performance and driving clinical outcomes.
• Strong understanding of healthcare delivery systems, population health, and value-based reimbursement models.
• Excellent communication, collaboration, and relationship-building skills.

Responsibilities

• Lead and support physicians and APPs in achieving quality, patient experience, and cost-efficiency goals.
• Monitor and analyze provider performance metrics; implement improvement plans.
• Facilitate peer review processes, clinical education, and professional development.
• Promote evidence-based practices and adherence to clinical guidelines.
• Collaborate with operational leaders to optimize care delivery models and workflows.
• Develop and maintain strategic relationships with external healthcare entities (SNFs, rehab centers, hospitals, specialists).
• Serve as primary clinical liaison to ensure seamless care transitions across the continuum.
• Partner with community providers to align on patient outcomes, quality, and cost containment.
• Represent MPG in regional forums, coalitions, and initiatives.
• Contribute to regional strategies for population health management and value-based care transformation.
• Identify opportunities for innovation in care delivery (telehealth, home-based care, integrated pathways).
• Support development and execution of clinical programs tailored to regional needs.
• Ensure compliance with regulatory standards and internal policies.
• Lead quality improvement initiatives and support risk management.
• Collaborate with data analytics teams for insights and continuous improvement.
• Work with Chief Medical Officer of the Florida region to align clinical strategies.
• Support practices and providers in accurate and timely documentation.
• Work with Regional VPs on operational and clinical initiatives.
• Participate in state and national MPG committees and special projects.
