Job summary

Sequoia Vista at Kaweah Manor is seeking a registered nurse licensed in California to manage the RAI and care management processes from admission to discharge. You will coordinate MDS coding, data entry, and compliance with federal, state, and certification guidelines under the direction of the Director of Nursing and VP of Clinical Reimbursement.

The ideal candidate has SNF experience, knowledge of MDS 3.0, OBRA, and Medicare/Medicaid reimbursement, and a thorough understanding of the care

Qualifications

• RN or LVN license in the state of practice.
• Minimum 2 years of nursing experience in a Skilled Nursing Facility.
• Knowledge of MDS 3.0, RAI, OBRA, and minimum data set.
• Understanding of the care planning process.
• Experience with MDS coding and data entry.
• Familiarity with federal and state regulatory guidelines.

Responsibilities

• Collaborate with the Interdisciplinary Team to schedule MDS assessments and care plan reviews.
• Assist with decisions on additional MDS assessments based on clinical criteria.
• Coordinate daily stand-up meetings, review resident care, and set assessment dates.
• Comply with federal and state regulations for the RAI process.
• Monitor MDS and care plan documentation in the medical record.
• Maintain current MDS status and accurate data entry in computerized systems.
• Complete accurate coding of the MDS from records and interviews.
• Other duties may be assigned at any time.
