Job summary

Rancho Seco Care Center is seeking an experienced MDS nurse to provide expert leadership and support for organization-wide initiatives in NorCal—Stockton and Sacramento. The role focuses on MDS completion, QA/QI, and RAI alignment to drive 5-Star quality outcomes.

The incumbent will audit assessments, educate MDS coordinators, and collaborate with the IDT to maintain regulatory compliance and optimal resident care. Suitable for experienced RNs with supervisory exposure.

Qualifications

• Must possess a current, active RN license in the state.
• 3 years’ RN experience, with supervisory exposure preferred.
• Experience with MDS completion and RAC considerations.
• Knowledge of PDPM, QAPI, and quality reporting programs.

Responsibilities

• Overseeing resident assessment processes across facilities per regs.
• Auditing MDS submissions and related clinical documentation.
• Reviewing records for coding accuracy and supporting docs.
• Interpreting rules for care-based reimbursements and quality reports.
• Providing MDS Coordinator education and guidance, coordinating care planning.
• Assist IDT with care planning and RAI-related guidance.

Position Description

We are looking for an outstanding MDS nurse to work closely with our facilities in NorCal- Stockton and Sacramento. This consultant will be responsible for providing expertise, leadership, modeling, and support for organization-wide initiatives. He/she will also work on the implementation and or evaluation of the facility’s Quality Measurement and Performance Improvement program to ensure quality of resident care outcomes.

Qualifications

• Must possess a current, active license to practice as a registered nurse in this state.
• 3 years’ experience as a registered nurse, one of which includes supervisory experience, preferred.
• Experience with MDS completion and process.
• Resident Assessment Coordinator (RAC) Certification is preferred.
• Understanding of computer technology, including Electronic Health Record (EHR) systems

Duties and Responsibilities

• Overseeing assigned facilities’ comprehensive resident assessment process in accordance with state and federal regulations.
• Auditing completion of MDS assessments and any supporting assessments or clinical documentation.
• Reviewing medical records for the presence of supporting documentation for all items coded on the MDS. Provide education as necessary.
• Interpreting rules, regulations and coverage guidelines and acting as primary resource for problem solving regarding care-based reimbursement systems and quality reporting program.
• Provide consultative support and training to MDS Coordinators within the assigned facilities, to coordinate and guide resident-centered care.
• Assist the Interdisciplinary Team with Care Planning.
• Provide additional guidance related to clinical documentation and coding as it relates to the Resident Assessment Instrument (RAI).
• Provide support and guidance to assist assigned facilities to achieve and maintain 5-Star Quality Measure Rating.
• Auditing and monitoring of RAI timeliness and accuracy of information in furtherance of regulatory standards.
• Support resident care by identifying trends and developing processes and action plans. Must be able to perform and present Root-Cause-Analysis.
• Reviews and audits data to support assigned facilities’ obligation to accurately capture diagnosis codes in accordance with PDPM guidelines.
• Provide on-going education, support, and make recommendations to optimize quality of care delivery.
• Attend and support assigned facilities’ Quality Assurance Performance Improvement Programs (QAPI).
• Work as an advocate for MDS Coordinators in relationship to the facility leadership, and the Interdisciplinary Team (IDT).
• Maintain the most current knowledge of State and Federal guidelines and regulations related to the RAI and reimbursement.
• The Regional MDS Nurse will assist the facility to prepare for the surveys and should be available to assist during the survey.
