Job summary

Center Management Group seeks a Regional MDS Coordinator RN to oversee the MDS/RAI process across multiple facilities in a hybrid remote/in-person setting. This role requires strong MDS/PDPM expertise, CMS/state regulatory knowledge, and a willingness to travel, train, and lead cross-site teams.

The position emphasizes mentoring facility staff, ensuring timely, compliant assessments, and collaborating with administrators to optimize quality outcomes and reimbursement accuracy.

Qualifications

• Current, active RN license.
• Minimum 3 years of MDS/RAI experience in long-term care; regional or multi-site experience preferred.
• In-depth knowledge of PDPM, Medicare/Medicaid, and case-mix reimbursement.
• Ability to travel regularly between assigned facilities.
• Excellent communication and leadership abilities.

Responsibilities

• Provide oversight, guidance, and support to facility-based MDS coordinators.
• Ensure accurate and timely completion of MDS assessments in compliance with CMS and state guidelines.
• Train and mentor facility staff on MDS, care planning, and PDPM requirements.
• Audit assessments and documentation to ensure accuracy and compliance.
• Collaborate with administrators, DONs, and interdisciplinary teams to drive quality outcomes and reimbursement accuracy.
• Monitor regulatory updates and provide education to facility teams.
• Assist with survey preparation and serve as a resource during state/federal inspections.
