Job summary

WECARE NURSING is seeking a Regional MDS Coordinator RN to oversee the MDS/RAI process across multiple facilities. Ideal for an experienced RN with PDPM knowledge who enjoys travel, training, and leadership.

The role involves mentoring staff, ensuring CMS/state compliance, and coordinating with Administrators and interdisciplinary teams to optimize reimbursement and quality outcomes. Hybrid remote/in-person position in New Jersey.

Qualifications

• Current, active RN license (NJ/PA or compact state).
• Minimum 3 years MDS/RAI experience in long-term care.
• In-depth knowledge of PDPM, Medicare/Medicaid, and case-mix reimbursement.
• Strong analytical, organizational, and teaching skills.
• Ability to travel regularly between facilities.
• Excellent communication and leadership abilities.

Responsibilities

• Provide oversight, guidance, and support to facility-based MDS coordinators.
• Ensure accurate and timely completion of MDS assessments in CMS/state guidelines.
• Train and mentor facility staff on MDS, care planning, and PDPM requirements.
• Audit assessments and documentation for accuracy and compliance.
• Collaborate with administrators, DONs, and interdisciplinary teams to drive quality outcomes and reimbursement accuracy.
• Monitor regulatory updates and provide education to facility teams.
• Assist with survey preparation and serve as a resource during inspections.
