Job summary

North Shore Healthcare is seeking a Regional MDS Coordinator (RN) to oversee Medicare technical oversight and managed care for a cluster of centers in the Sheboygan/Fon Du Lac area. The role requires RN licensure, PDPM experience, and CMAC/AANAC RAi certification, with significant attention to MDS planning and care coordination.

Qualifications

• Must be a Registered Nurse with an active license in the state.
• RAI Certification via CMAC or AANAC or willingness to complete.
• 3+ years of PDPM experience and working knowledge of state payor systems.
• 3+ years of skilled nursing experience with RAI/CMI focus.
• Knowledge of the MDS assessment cycle and assessment information.
• Computer proficiency with Microsoft products.

• This is a manager position overseeing multiple local Centers.

Summary of position

Nurse will assume the Medicare technical oversight and managedcare responsibilities onsite and or in a remote capacity in a cluster ofcenters. They will be accountable for coordinating and overseeing the fullcollaborative, interdisciplinary assessment and care planning process. Thisprocess includes managing the completion of the comprehensive residentassessments; care coordination and planning; staff teaching; and adherence tothe RAI guidelines. Conducting meetings,tracking payors, reviewing skilled criteria and handling the technical areas ofMedicare and Managed Care are core job components. This is a manager positionoverseeing multiple local Centers.

Requirements of the position

Required Education and Experience

• Currentlicense to practice in the State as an RN
• RAICertification via CMAC or AANAC/willing to complete
• 3+years of PDPM experience and working knowledge of state payor systems
• 3+years of skilled nursing experience with RAI/CMI focus
• Workingknowledge of the MDS assessment cycle and assessment information
• Computerproficiency with Microsoft products
