Aspire Senior Living is seeking a Regional MDS Coordinator to oversee MDS assessments across our skilled nursing facilities in Missouri. This role requires coordinating interviews, reviewing records, and educating the care team to maintain MDS compliance and high-quality resident care.

The ideal candidate will be a registered nurse with a strong understanding of MDS regulations, excellent communication, and organizational skills to support seamless care transitions across facilities.

Qualifications

• Bachelor's degree in Nursing or related field is required.
• Current RN licensure in Missouri is required.
• Experience in a skilled nursing setting, especially MDS coordination.
• Strong knowledge of MDS regulations, guidelines and best practices.

Responsibilities

• Coordinate and conduct accurate and timely MDS assessments for residents in our skilled nursing facilities
• Review medical records and conduct resident interviews to gather necessary information
• Provide education and support to our care team to ensure MDS compliance
• Identify and report concerns or issues related to resident care and quality of life
• Collaborate with other departments to ensure seamless transitions and comprehensive care

About the Role:

As a Regional MDS Coordinator with Aspire Senior Living, you will be responsible for coordinating and maintaining accurate and timely Minimum Data Set (MDS) assessments for residents in our skilled nursing facilities. This includes conducting resident interviews, reviewing medical records, and providing education and support to our care team. Your expertise and attention to detail will ensure that our residents receive the highest level of care possible.
