Job summary

AdventHealth seeks a Nursing leader to manage Community Care operations, ensuring clinical and regulatory compliance while driving quality outcomes across teams in Florida. You will oversee staffing, performance, documentation, and collaboration with physicians and community partners to achieve program goals.

The role requires a BSN, RN licensure, and 4+ years in clinical settings with supervisory experience.

Qualifications

• RN licensed and eligible, Bachelor of Nursing required.
• 4+ years in a clinical setting with supervisory experience.
• Strong documentation, regulatory and quality-monitoring knowledge.

Responsibilities

• Manage Community Care operations and ensure clinical and regulatory compliance.
• Monitor productivity and outcomes, identify root causes and act on improvements.
• Document care plans accurately in medical records with confidentiality.
• Prescribe, delegate, and coordinate patient care activities to meet plans.
• Develop relationships with physicians, clinics and community partners.
• Contribute to strategic plans and budget input for the program.
• Oversee hiring, scheduling, orientation, training and performance.
• Lead team meetings, case conferences and care improvement guidance.
• Consider safety, effectiveness, and cost factors in service delivery.
• Perform additional duties as assigned.

Job Description

• Manages Community Care program operations and ensures compliance with clinical standards, regulatory requirements, and quality monitoring processes.
• Monitors productivity and outcome metrics, identifies root causes of performance issues, and implements corrective actions.
• Ensures accurate, timely, and comprehensive documentation of patient care plans and responses in medical records while maintaining confidentiality and legal compliance.
• Prescribes, delegates, and coordinates patient care activities to achieve established care plans and priorities.
• Develops and fosters relationships with physicians, clinics, universities, and community partners to support program goals.
• Implements strategic plans and provides input on program development and budgetary needs.
• Oversees hiring, scheduling, orientation, training and performance management of Community Care staff.
• Attends and conducts team meetings, case conferences, and provides guidance for care improvement.
• Considers safety, effectiveness, and cost factors in service delivery and resource management.
• Performs other duties as assigned.

• 4+ years of experience in a clinical setting [Required]
• 2+ relevant clinical experience which would provide the required knowledge, skills and abilities [Required]
• 2+ supervisory or management experience [Required]
• 2+ community clinical experience such as home health, hospice, mental health, or community social work [Preferred]
• 2+ management experience in a community setting [Preferred]

Additional Information

An equivalent combination of education and relevant work experience may be considered in lieu of the stated degree requirement: Bachelor’s of Nursing degree AND 4+ years of experience in a clinical setting OR 8+ years of experience in a clinical setting
