Job summary

Apploi is seeking a Director of Care to provide clinical leadership across approximately 10 skilled nursing facilities in Connecticut and Massachusetts. This hands-on role requires an RN or experienced nursing leader who can drive consistent standards of care across multiple facilities.

The Director of Care will partner with DONs, Administrators, and regional leadership to improve clinical outcomes, regulatory compliance, and resident care while traveling regularly between CT and MA facilities.

Qualifications

• Current RN license in good standing; multi-state licensure is a plus.
• BSN preferred.
• Significant clinical leadership in skilled nursing / long-term care.
• Experience overseeing multiple SNFs or nursing facilities.
• Prior DON/Regional leadership experience strongly preferred.
• Strong understanding of CMS and state regulatory requirements.
• Knowledge of QAPI, MDS, care planning, infection prevention, quality measures, and risk management.
• Excellent communication, leadership, organizational and problem-solving skills.
• Willingness to travel regularly between facilities.

Responsibilities

• Provide clinical oversight across multiple skilled nursing facilities.
• Partner with DONs, Administrators, and facility leadership to maintain high resident care standards.
• Conduct regular facility visits, clinical assessments, and operational reviews.
• Monitor quality measures, rehospitalizations, falls, infections, wounds, medications, and other outcomes.
• Identify clinical risks and develop improvement plans.
• Support CMS/state surveys, regulatory compliance, and plans of correction.
• Evaluate nursing practices, documentation, care planning and clinical processes.
• Coach and mentor DONs and nursing leadership.
• Promote standardized policies and best practices across the organization.
• Collaborate on staffing, retention, education, and clinical workforce development.
• Assist with QAPI programs and data analysis for improvements.
• Serve as clinical resource during complex resident care and operational challenges.
• Develop and implement standardized clinical programs across the organization.
• Travel regularly between CT and MA facilities.
